Dear parent/guardian,
once the pre-filled PDF entry form has been received, you must complete the registration as follows:
1) complete, sign and send the pre-filled registration form attached to the confirmation-mail to the e-mail margherita@sporteventi.it (Margherita Lombardo – Responsible for the Secretariat office)
2) send the confirmation of payment via bank transfer to the same e-mail (margherita@sporteventi.it). ATTENTION: indicate as the reason for the back transfer ONLY the NAME and SURNAME of the participant, and address it to ASD SPORTEVENTI, on bank account no. 104571205890 at Banca Popolare dell’Alto Adige, Branch (filiale) of Vittorio Veneto (Italy, TV province), IBAN: IT02B0585662190104571205890 and SWIFT code: BPAAIT2B104.
If the registration form and the confirmation of the bank transfer are not received by Sporteventi within seven (7) days of this email, the reservation will be deemed to have lapsed and will no longer be considered by ASD Sporteventi.

3) send to the e-mail margherita@sporteventi.it as soon as possible, and in any case at least three (3) days before the start of the booked camp, the medical certificate (Family Doctor's Clean Bill of Health) where a doctor recognized by the National Health System, having examined the child/teenager (participant/registrant), states:
- not having diagnosed any clinical signs restraining the practice - in competition or not - of football/soccer and specifically of the football/soccer sport activity named as “AC Milan Junior Camp” which is a not-competitive-summer-football camp for children/boys aged 6-16
- the electrocardiogram (ECG) is not needed / The electrocardiogram (ECG) doesn’t show any sign of of detectable illness / disease;
- that the subject does not show any sign of detectable contagious illness / disease.
The medical certificate, not older than one year from the beginning of the camp, must cover the entire period of the camp, otherwise the registration will be cancelled. Nobody will be admitted to the camp without the Health Certificate or with a wrong one. The medical certificate have to be sent by mail to margherita@sporteventi.it as soon as possible and in any case at least three days before the reserved camp starting day;
4) send to the email margherita@sporteventi.it a copy of the participant/registrant’s identity card or passport;
5) send to the email margherita@sporteventi.it a copy of the participant/registrant’s health card if resident in Italy;
6) send the duly completed and signed form “Self-certified consent form responsible for medical services” to the e-mail (margherita@sporteventi.it), attaching a copy of the identity card or passport of the participant’s parent/guardian.
7) send to the email margherita@sporteventi.it the following forms duly completed and signed:
- Authorization for the use of audio, video recordings and photos
- Privacy notice according to GDPR 2016/679
- Sporteventi organization chart
- Information and rules
- Corporate Code of ethics and conduct
Within three (3) days of receiving the signed registration form and the copy of the bank transfer, we will send you an email confirming the registration and containing details on the overall cost of registering for the camp.
Finally, we will send you, again via mail, seven (7) days before the start of the chosen Milan Junior Camp, a detailed program of the week.
